Listed here are the forms that you will receive in our booking confirmation email after making a booking; they are listed here to download again if you wish. You will need to return the first three (ie 1 x Booking Form, all Medical Forms and the Disability List) to us.

1. Schools Booking Form
>> Schools Booking Form

Every school booking will need to complete and return 1 x Booking Form. This is required at your earliest convenience following your booking. Our cancellation terms are noted on the reverse of the Booking Form.

2. Medical Form
>> Medical Form

EVERY person attending the Calvert Trust will need to complete, sign and return one of our Medical Forms. This includes a dietary requirements sheet, where you can list allergies, intolerances or any special dietary requests. We require these at least 2 weeks before the start of your visit. If you are sending these by post, make sure you TAKE A COPY and retain for your own reference.

3. Disabilities List
>> Disabilities List

Please complete and return a Disability List at your earliest convenience. It is really important to include the weight for each person on this sheet, as this is used at the stables. Remember to do ‘Save as’ before you fill out and return via email to us.

4. Group Organisers/Teachers Information Sheet
>> Group Organisers/Teachers Information Sheet

This explains things a little more about your booking, for instance the terms of your booking, the facilities at the Centre and what to expect when you are here.

5. Visitors and Parents Information Sheet
>> Visitors and Parents Information Sheet

This explains a little bit more about your stay here – for example what group members will need to bring, such as kit lists and logistics of the visit, such as arrival/departure times and Directions (see below also). The Visitor Information sheet will be useful to give to your group members, parents of pupils and teachers who are attending.

6. Example Risk Assessment for Pool
>> An example of a Risk Assessment which may assist you

The Lake District Calvert Trust does not provide lifeguards for our hydrotherapy pool. Therefore anyone supervising children or vulnerable adults in a professional or otherwise non-domestic capacity must have completed their own risk assessment. We will require supervisors to complete an induction with our staff and to complete a declaration confirming a risk assessment is in place before access to the hydrotherapy pool can be given. For your information a pool side alarm is present and Calvert Trust staff responding to an alarm have received training to the level of the RLSS Emergency Response Swimming Pool qualification.

  • Getting to the Centre
The Lake District Calvert Trust is situated at Little Crosthwaite, just 4 miles NW of Keswick on the A591. For SatNav users our postcode is CA12 4QD.

Exit the M6 at junction 40 and take the A66 towards Keswick. Remain on the A66, just past Keswick until you reach a large roundabout. Take the A591 exit (signposted Bothel and Carlisle) and head towards Carlisle and continue along this road for 3 miles. The Calvert Trust Centre is on the right.

  • Arrival and Departure Times
You are welcome to arrive at the Centre between 3 – 5 pm, however we cannot guarantee rooms will be ready until 4pm. Evening meal will be ready at 6pm. We ask that you depart the rooms by 10am on your departure day, following breakfast.
  • Parking and Coach Transport
Parking is very limited on site. We do have space for 16/18-seater minibuses to park during your stay, however we will not be able to accommodate parking spaces for larger coaches, other than for drop-offs and pick-ups.
